ABOUT YOUR VISIT

  • Please arrive 15 minutes prior to your scheduled visit to complete your medical information.
  • Bring a list of your current medications
    • Include dosage, frequency
  • Contact imaging facility to have CT disc and report delivered to office prior to scheduled appointment
  • HMO insurance requires referral
  • Insurance card and Photo ID (current)

  • Please discuss any financial difficulties you may anticipate with our office or billing manager. Her name is Susan Lohr and she has many years of experience working with insurance companies and understands complicated coding and billing situations.
  • Regarding authorizations for tests, procedures or surgeries – the authorization is not a guarantee of payment and applies only to services rendered by or ordered by our physician. Facility fees are separate and any questions you may have regarding facility reimbursement should be directed to the specific site
