Our office is permanently closing 12/31/21.  We are happy to help you with any immediate problems with the understanding that after 12/ 17 /21 further care will have to be provided by your PCP or one of the regional Ear, Nose, & Throat physicians.


Before You Visit
  • Check your symptoms. If you have a cough, fever over 99.6 degrees Fahrenheit, or shortness of breath, please call your primary care physician.

About Your Visit
  • Please arrive 15 minutes prior to your scheduled visit to complete your medical information.

  • Bring insurance card and Photo ID (current)

  • Medical Park is dedicated to providing you with the highest quality, cost effective medical care. We accept cash, check and all major credit cards.

  • Bring a list of your current medications

    • Include dosage and frequency

  • Contact imaging facility to have any CT discs and reports delivered to office prior to scheduled appointment

  • Obtain any required insurance referrals

Protecting you and our staff from COVID-19

  • Call the office 570-523-1163 from the parking lot when you arrive. A staff member will meet you to do screening questions and take your temperature.

  • You will be expected to wear a mask. Only one person may accompany you to the examination room.

  • Bring your medication list–must include name, dosage, and frequency. This list will not be returned.

  • If you are a new patient, we strongly recommend you fill out our health history questions online through our portal. If you need information on how to access the portal, please call the office. If you choose to do your paperwork upon arrival, we will give you the documents to fill out in your car. Plan to arrive 40 minutes prior to scheduled appointment time to fill out the documents. If you do the forms online, you should arrive 10 minutes prior to the appointment.

Co-payments, deductibles and non-covered charges are due at time of visit.

We participate with most insurance plans. However, patients should contact their insurance carrier prior to the visit to discuss benefits.

Please discuss any financial difficulties you may anticipate with our office or billing managers. They have many years of experience working with insurance companies and understand complicated coding and billing situations.


Regarding authorizations for tests, procedures or surgeries – the authorization is not a guarantee of payment and applies only to services rendered by or ordered by our physician. Facility fees are separate and any questions you may have regarding facility reimbursement should be directed to the specific site.
